Organized for Life is a national campaign designed to:

> provide the general population with pro-life material through which they will have a better understanding of life issue
> provide resources which provide information for direct pro-life involvement

Field Agent Report Form

During and after the conversation with the residents, information needs to be gathered, for them and for us.

Organized for Life field agents shall carry a clipboard with the following materials attached to it:

- Survey
- Petition to end abortion
- Separate sheet for signing them up. We need to be able to follow-up, to respond to them and encourage them to get involved.
- Field Report form (at the back, always)

Field Report after holding a life-issue discussion with a pro-choice citizen (the Responder). At conversation’s end, Field Agents should ask the Responder to rate how he/she felt about current abortion law both before and after the conversation took place.

No report form needs to be completed if the person is pro-life or if no one answers the door.
